I have seen a PCV valve cause a hesitation if the orific is too big! As redvett94 said.

Ok, have ruled out MAF, PCV valve and fuel filter, anyone think it could be a clogged Cat?

If you can get a MAF of a friends Truck I would try that! It sounds like a MAF problem to me.

There are a few signs of a clogged cat. First is the odor in the exhaust smelling kind of like sulphur or rotten eggs. As it progresses it will make a loud hissing noise from exhaust getting through a clogged system. Eventually it will barely run past an idle.

Cats are usually OK when cold and get worse after you drive for a bit. And if you have a infrared temp gauge you can check the temp of the cat. It should be cooler at the input and hotter at the output. If it is the same temp throughout the cat is no good.JMO
